Replace the housing as well as the light source
Integrated LEDs
Each replacement brings its own LED board, optics and lens. This package is a pair of complete lamps, not a pair of loose replacement bulbs.
Built for the bumper location
UV-coated polycarbonate lenses and IP67-rated housings protect the assemblies. Built-in transient protection addresses voltage spikes at the circuit board.
A short job with a plastic trim tool
Release the original housing, expose and remove its bulb, then connect and test the new assembly before snapping it into place. FORM lists a typical installation time of 5–10 minutes and no modification for the supported factory arrangement.
If an LED does not illuminate during the test, the manufacturer instructs you to disconnect it, reverse the plug orientation and test again.

Is one purchase enough for both sides?
Yes. FL0046 is supplied as a left-and-right pair.
Do I need separate LED bulbs?
No. The light sources are integrated into the supplied replacement housings.
Will this fit a modified rear bumper?
The product is designed for the factory F-150 lamp openings and connections. Confirm an aftermarket bumper retains those features before ordering.
What warranty applies?
FORM supports these assemblies with its two-year manufacturer warranty. This FORM product is not covered by the separate lifetime lamp wording used on some Diode Dynamics products. Contact BTS for help with a warranty concern.
